§ 9:962. Application for teen club license.

  • (a)

    An applicant for a teen club license shall submit an application to the alcoholic beverage control office along with the annual nonrefundable license fee of two hundred fifty dollars ($250.00) per application. The application shall include the following information:

    (1)

    The names and address of the owner, lessor and lessee of the property;

    (2)

    The name and address of every owner, officer, and managing agent of the teen club, and of such other employees that are known at the time of the application;

    (3)

    The name of the security personnel to be employed to provide security for the teen club;

    (4)

    Detailed plans and drawing of the teen club and adjoining areas showing the:

    a.

    Dance floor,

    b.

    Waiting area for persons seeking admission,

    c.

    Parking areas,

    d.

    All restrooms, coat rooms, game rooms, refreshment areas, and all other spaces accessible by patrons, with plans to secure those areas as may be required under section 9:973, and

    e.

    All interior and exterior doors and windows;

    (5)

    A binder showing the availability of the insurance required by section 9:972.

    (6)

    The alcoholic beverage control office shall reject any application which does not conform to the requirements of this section.

    (7)

    The alcoholic beverage control office may conduct a criminal history background examination on any person listed on the application.
