§ 6:477. Application.

  • Any person desiring to establish, maintain or operate a bakery shall make application for a permit, in writing, at the office of the health unit or a blank furnished by such office. Such application shall set forth the name and residence of the applicant if an individual, and the names and residences of all members if a firm, or the president, vice-president and secretary if a corporation, together with the location of the place for which the permit is desired. The application shall also state the maximum number of persons to be employed in the bakery, the number of rooms or apartments therein, and any other facts concerning the proposed bakery which the board of health may require to have stated in the application.
