§ 10:417. Application for air ambulance permits.

Applications for permits shall be made to the EMS department and the finance director upon forms prescribed by him, and shall include:

(1)

The name and address of the owner or operator of the air ambulance service;

(2)

A description of the aircraft, including the manufacturer, model, year of manufacturer and the FAA identification number;

(3)

The location and description of the place or places from which it is intended to operate;

(4)

Such other information as the department of EMS shall find reasonably necessary to make a fair determination of whether the provisions of this chapter have been complied with.
