§ 8:14. Application for license.

Each person who is required to obtain a license under the provisions of this part shall make application in writing to the mechanical official on forms provided by him. The application shall be signed by the applicant under oath and shall contain the following information:

(1)

The full name, sex and age of the applicant.

(2)

Residence address at the time of application.

(3)

The name under which the business is to be conducted.

(4)

Location of place of business for which the license is sought.

(5)

The experience the applicant has had in the business or occupation for which the license is sought.

(6)

Class of license being sought.

(7)

Residential telephone number.
