§ 1:450. Medical information and examination.

  • (a)

    For purposes of ascertaining the presence of preexisting conditions relative to the right to receive disability retirement benefits from the employees' retirement system of the city-parish, as a condition of employment and as provided herein, all prospective employees shall provide requested medical information and undergo prescribed medical examinations prior to employment.

    (b)

    The personnel department shall require each prospective employee to complete a medical questionnaire, which questionnaire shall be approved by the board of trustees of the retirement system.

    (c)

    The personnel department shall require each prospective employee to undergo medical examination by physicians chosen by the department. With respect to each prospective employee, the examining physician shall complete an examination form, which form shall be approved by the board of trustees of the retirement system.
